Dear Manufacturers, Enclosed is a short survey to assess the Product
Approval Administrator's (Ted Berman and Associates - TB&A) performance
for the August 2009 meeting cycle. Please send your responses to
suzanne.davis at dca.state.fl.us by close of business on September 9, 2009.
Survey Instructions: This survey consists of only two portions: 1)
Select a rating of either: Excellent, Good, Fair, or Poor. 2) Add any
comments you can think of regarding how TB&A performed, and/or how they
may improve performance in the future. __________ Note: Please be
mindful that the BCIS system (floridabuilding.org) is not a part of
TB&A's performance. Feel free to comment on the system, however, do not
include it as a factor in the rating of the Product Approval
Administrator's performance. ****************************************
Survey Question for August 2009: For the meetings held in August, 2009,
consider TB&A's overall performance, including telephone and system
support, application review, emails describing the results of
application review, recommendation to the Product Approval Oversight
Committee (POC) and timely carrying out of the final Commission Vote on
your application(s), would you rate TB&A's performance as: 1) Choose a
