Thanks for your reply...and others.
 
I reviewed specific ESR for ICF walls in question. (ESR is not part of
the product approvals for State or Miami-Dade.  Don't know why.  ESR has
a lot more information than can be downloaded at either website.)  There
is a section in the ESR that states termite prevention must be provided
as required by code.  ESR does not mention anything about an option for
termiticide.  Maybe this is provided by manufacturer when requested by
purchaser.  Interestingly, the ICF manufacturer has a mesh material that
they use for termite prevention on some of their walls.  It was not
listed as an option for the ICF walls noted on plans under review.  At
this point, the responsibility for showing code compliance falls on the
design professional.  Again, thanks to all that replied on this topic.

Many of the producers of foam plastics intended for use near grade are
infusing the product with termiticides during the manufacturing process.
Certification may be obtained from the supplier or the manufacturer.

I've been involved in that type of construction and the manufactures
does not allow that. I would suggest getting the manufactures spec
relating to that issue. Also the code is clear. If you need more let me
know
